ABSTRACT Purpose: To evaluate the safety and 12-month effect of treatment with pattern scanning laser photocoagulation for ocular surface squamous neoplasia in a low-resource setting with extremely limited access to an operating room. Methods: Adult patients with a clinical diagnosis of ocular surface squamous neoplasia underwent a complete ophthalmologic examination. After topical anesthesia and instillation of toluidine blue 1%, the lesion was treated using pattern scanning photocoagulation for a duration time that varied from 20 to 100 ms and power from 600 to 1,800 mW. Patients were examined on a weekly basis for the first month and underwent weekly retreatment of the remaining lesions, as necessary. Patients had a minimum follow-up of 12 months. Results: Thirty-eight patients (38 eyes) were included. All patients had clinical ocular surface squamous neoplasia that was confirmed by impression cytology. The age of patients ranged from 40 to 83 years (average: 65.5 years) and 28 of them were males (74%). The patients were divided into two groups: group I (immunocompetent) and group II (immuno­suppressed). In group I, 23 patients (74%) presented complete response with lesion control after laser treatment alone. In group II, two of seven patients (28%) showed treatment response during the follow-up. The average number of treatments was 2.5 (one to six laser treatments). Procedures were well tolerated. Conclusion: Short-term results of the laser photocoagulation approach for the treatment of ocular surface squamous neoplasia conjunctival lesions were favorable, with a 74% success rate observed in immunocompetent patients. This novel strategy is a less resource-intensive alternative that could demonstrate its usefulness in settings with shortages in operating rooms and in recurrent cases. Studies with longer follow-ups and larger sample sizes are warranted to confirm our findings and evaluate the effectiveness of laser treatment in association with topical chemotherapy.


RESUMO Objetivo: Avaliar a segurança e o efeito de 12 meses de tratamento com fotocoagulação pelo pattern scanning laser para neoplasia escamosa da superfície ocular em um ambiente com poucos recursos e acesso extremamente limitado a um tratamento cirúrgico. Métodos: Pacientes adultos com diagnóstico de neoplasia escamosa de superfície ocular foram submetidos a exame oftalmológico completo. Após anestesia tópica e instilação de azul de toluidina 1%, a lesão foi tratada com laser por um tempo de duração que variou de 20 a 100 ms e potência de 600 a 1800 mW. Os pacientes foram examinados semanalmente durante o primeiro mês e fo­ram retratados semanalmente das lesões restantes, conforme necessário. Os pacientes tiveram um seguimento mínimo de 12 meses. Resultados: Trinta e oito pacientes (38 olhos) foram incluídos no estudo. Todos os pacientes apresentaram neoplasia escamosa da superfície ocular clínica, confirmada por citologia de impressão. A idade dos pacientes variou entre 40 e 83 anos (média de 65.5 anos) e 28 deles eram do sexo masculino (74%). Os pacientes foram divididos em dois grupos: Grupo I (imunocompetente) e grupo II (imunossuprimido). No grupo I, 23 pacientes (74%) apresentaram resposta completa com o controle da lesão após o tratamento com laser. No grupo II, dois dos sete pacientes (28%) apresentaram resposta ao tratamento durante o acompanhamento. A média de aplicações de laser foi de 2,5 (1 a 6 aplicações). Os procedimentos foram bem tolerados. Conclusões: Os resultados a curto prazo da abordagem de fotocoagulação a laser para o tratamento das lesões conjuntivais de neoplasia escamosa de superfície ocular foram favoráveis, com uma taxa de sucesso de 74% observada em pacientes imunocompetentes. Essa nova estratégia é uma alternativa menos intensiva em recursos que pode demonstrar sua utilidade em ambientes com escassez de salas cirúrgicas e em casos recorrentes. Estudos com acompanhamentos mais longos e amostras maiores são necessários para confirmar nossos achados e avaliar a eficácia do tratamento a laser asso­ciado à quimioterapia tópica.

RESUMEN Fundamento: el carcinoma epidermoide de seno maxilar es de poca frecuencia, capaz de provocar compromiso estético-funcional por lo que su diagnóstico temprano permitirá al paciente un mejor pronóstico. Objetivo: exponer aspectos clínicos del carcinoma epidermoide de seno maxilar. Caso clínico: paciente masculino de raza blanca, 64 años de edad, con aumento de volumen en región orbito-cigomática izquierda de un mes de evolución. La tomografía axial computarizada de senos paranasales reveló imagen heterogénea sugestiva de tumor de seno maxilar del lado izquierdo. El diagnóstico histopatológico reportado fue de carcinoma epidermoide bien diferenciado. Se instauró el tratamiento con quimioterapia y radioterapia concurrente. El motivo de mostrar este caso consiste en aportar experiencias que permitan el diagnóstico temprano de la enfermedad y así mejorar el pronóstico para el paciente. Conclusiones: un examen clínico minucioso conducirá a un diagnóstico precoz de estas enfermedades que presentan una evolución tórpida y carecen de signos patognomónicos sobre todo en sus estadios iniciales. Es de vital importancia el papel protagónico que tiene la Atención Primaria de Salud para la detección del cáncer el cual constituye una de las primeras causas de muerte en Cuba.


ABSTRACT Background: squamous cell carcinomas of maxillary sinus are rare. It causes esthetic-functional compromise. Therefore, its early diagnosis will allow the patient better prognosis. Objective: to expose clinical aspects of squamous cell carcinoma in maxillary sinus. Case report: a clinical case is reported from 64-year-old white male patient with swelling over the orbit-zygomatic area that progressed over 1 month. The paranasal sinus CAT scan showed a heterogenic image suggesting a left maxillary tumor. The histopathological diagnosis reported a well- differentiated squamous cell carcinoma. Chemotherapy and radiotherapy can be used in this case. The scope of showing this case is to provide some aspects that allow the early diagnosis of the aforementioned disease as well as to improve the patients' prognosis. Conclusions: the deep clinical examination in patients will allow a soon diagnosis of these pathologies that present a torpid evolution lacking pathognomonic signs mainly in first stages. It is of paramount importance the role of the Primary Health Service in the early detection of cancer, which constitutes one of the main causes of death in Cuba.

Occurrence of concurrent multiple primary malignancies with different histopathological presentations of the same organ at the same time is often not diagnosed and rarely reported in the literature. We present a case of multiple primary lung cancers with hepatic metastasis where the patient had a moderately differentiated adenocarcinoma of the right lower lobe and a moderately differentiated squamous cell carcinoma of the right upper lobe.

ABSTRACT

Introduction: Keratoacanthoma is a lesion that may mimic squamous cells carcinoma clinically and histologically and the distinction between both lesions has been a matter of discussion. Typical lesions consist of a firm dome-shaped nodule, 1 to 2 centimeters in diameter, with a horn-filled crater in its center. Objectives: To present it was selected an atypical case of a 43-year-old woman who had a solitary keratoacanthoma (KA) of the lower lip with a history of two years of evolution and mistreatment and to discuss the difficulties of the diagnosis process. Conclusion: Because of the rarity of the lesion and its similarity with squamous cells carcinoma the general dentist must explain to the patient that the clinical aspects are not conclusive of the diagnosis and a correct acquisition of the surgical specimen is crucial to an accurate histopathological interpretation and conclusive diagnosis.


Introdução: O ceratoacantoma é uma lesão que pode mimetizar clínica e histologicamente o carcinoma espinocelular, sendo discutível a distinção entre as duas lesões. As lesões típicas consistem em um nódulo firme, em forma de cúpula, com 1 a 2 centímetros de diâmetro e uma cratera central preenchida com queratina. Objetivo: Apresentar um caso atípico de uma mulher de 43 anos que apresentava uma lesão solitária de queratoacantoma no lábio inferior, com uma história de dois anos de evolução e erros de tratamento e discutir a dificuldade no processo de diagnóstico. Conclusão: Devido à raridade da lesão e sua semelhança com o carcinoma, o clínico geral deve explicar ao paciente que os aspectos clínicos não são conclusivos do diagnóstico e que a aquisição correta do espécime cirúrgico são cruciais para a análise histopatológica correta e para o diagnóstico conclusivo.

ABSTRACT

O carcinoma epidermóide apresenta moderada freqüência na cavidade bucal, podendo ser considerado um problema de saúde pública. O escopo deste trabalho é apresentar o caso de um paciente portador de carcinoma epidermóide em lábio inferior, o qual foi submetido ao tratamento cirúrgico. Foram discutidas as características clínicas, radiográficas e histopatológicas, freqüência, incidência e modalidades de tratamento.


The squamous cell carcinoma is frequent in the oral cavity, and is considered an important public health concern. The purpose of this study is to present the case of squamous cell carcinoma on lip that was submitted to the surgical treatment. We discussed the clinical, radiographic and histopathologic characteristics, frequency, incidence and treatment modalities.
